td crunching into fifth gear

I have a problem with a turbo diesel freelander crunching/grating into fifth gear.

Have had the gearbox out and replaced the synchromesh on 5th with genuine parts which cured the problem.

However one month on the problem has returned?

What else could cause this? Is this a common problem?
