Cam Chain Rattle

Right guys I've got a serious problem...

Initially I thought the rattle was only on idle but ive been and seen my local indie who can't look at the truck for a month he's so busy.

First things he said to check are chain tensioners and the sprocket bolt...

I've currently got the rocker cover off, I can see down beside the chain and see the tensioner but how can u tell if it's ok or not?

135k mate, I will add that the rattle is ten times worse with the aux belt on. With it off it's nowhere near as bad but still doesn't sound right?

Sounds like it could be a worn idler bearing.
