pack it with grease and then hammer a close fitting shaft into the hole.

Well seeing how much you've helped the forum in your 6 years of membership, I'll tell you.
Find a bolt or similar shaft that is a very good fit into the bush, then fill the bush with grease and gently but firmly push or tap the bolt/shaft into the bush which will force the bush out by hydraulic action.
