p38 2.5 diesel noisy tappets

Have you checked your oil?
Low oil, old thick oil, low oil pressure can all give a brief top end rattle before it starts to lubricate.
 
this has happened recently,only for a few seconds from cold then calms down,is this the beginning of the end?
vehicle has done 124,ooo miles

Just give it an oil service to start with, a noisy tappet is nothing to worry about normally, when the tappets start to rattle i normaly stick sum engine flush in the eng and run it for 10 mins befor i change the oil, if that doesnt cure it either chuck some new 1s in or get some tappet additive from your local motor factor and that normally shuts them up till next service, ultimatly new tappets should be fitted so excesive wear to the camshft is avoided but this doesnt mean that ur engine is ready for "the final stop" hope that helps.
 
Sure it's not just diesel clatter on cold mornings? My Transit sounds like a bag of spanners for the first 10 seconds on cold mornings, the P38 slightly less so.
 
Thinner oil an option? Mine's got better after a flush and additive but not as quiet as I'd like, I'm soon to flush one more time and possibly try a thinner oil, just not sure how thin to go.
