Oil cooler pipe self disconnects

My 2004 Disco 2 V8 has on two occasions lost the auto box fluid by the pipe uncoupling from the oil cooler. Do I need to replace the pipe only or the oil cooler as well. I need a repalcement in New Zealnd. (The Disco is a Japanese import as are the majority of NZ vehicles.) Currently on around 80k miles (130k km.)
 
Normally the connection needs cleaning so the connector snaps into place positively. Our son's oil cooler came off at traffic lights on his first run out after an engine change, once recovered I got him to thoroughly clean the pipe connector and make sure it went on positively.

The following two weeks he did 2000 miles with us in France with not a problem, and none since.

Mine collapsed inside the coupling,

I cut it off, cleaned the cooler and the pipe, smeared good quality flange sealant over the pipe and used a jubilee clip to hold the pipe on

That was a year ago, no further bother

Thanks for the advice. I will look at the coupling. It has been worked on by several garages over the last two years and it is possible it is far from clean.

Yeh it gets really gritty around that area. You should notice a very defined 'click' when it properly sits home. It's easy to think it's on when it's not.
