Greasing wheel bearings

How often should I grease my wheel bearings? I shall be doing the fronts shortly when I replace my brake discs but should I also do the rears while I'm at it? I had greased them when I refurbished my axles 5 years (45,000 miles) ago.

if you go wading ,as soon as you get home ,for normal driving once a year as part of a full oil change service

don't forget its not enough to jsut grease the outers, as a minumum you need to remove the outer race to get grease to the inner.
I whip the whole hub off and take the races out, degrease and check before greasing/replacing, my way does need a replacement seal but its £2...
