Valve Clearance Issue

Got the cam followers out, 1,2 and 5 are an absolute state, will try to get pictures up shortly. 2 and 5 have eaten a huge lump out of the sliders. All four hotspots fell out when I took the head of so ordered new.
Interestingly the green book and Haynes manual specify protrusion of 0.76 mm (0.003 in) and recess of 0.025 mm (0.001 in). But three thou is 0.076 mm!
 
Id be amazed if the cam escaped that sort of treatment with only minor damage. I'm just wondering how it's been caused on the rollers though. I'd be worried to put it back with new parts in case it occurs again. Could it be oil starved? I presume it's all ok for splash feed from the top and the cam bearings seem OK there is no odd flexing from a collapsed shell or anything?
 
It is an odd one. I would have thought if there were issues with the cam then there would be problems with all the lobes but there isn't. It could have been a bad batch of followers or possibly a Friday when the engine was overhauled (blue military block).
 
I would be a bit concerned about where all the ground up metal has gone. I would be tempted to get the sump off and have a look what has settled in it and have a look in the oil pump.

I'd personally want to take the camshaft oout and look myself, especially considering the amount of metal particles that will have been lurking around the bearings with that lot grinding away. Theres still a fair amount of work though to do to get it out sadly I think you have to disturb all the timing chest and belts which means the crank pulley needs to come off as well which is a bit of a PITA
