Slipping chain like noise under acceleration, and clutch problems

Hi all,
I have a 1995 2.5 dt manual with 230,000 miles on the clock which I have happily owned for 7 years, over the sat 2 years it has developed a noise which I can only describe as a loud chain slipping like noise when accelerating, does anyone have any idea what this could be and if so how to fix it???

Also last week my clutch pedal decided to stick down when I pressed it, I managed to get it up again but the other day I pressed it down to stop at a junction but nothing happened good job the brakes are good. Is this the clutch on its way out ??. I use the car on the farm daily and it's a great workhorse apart from the odd few niggles.
 
i never seen a chain jump the sprockets ,seen them break occasionaly ,draining some oil out of gear box and transfer box can help see were problem lies ,spiral pins on cross shaft or poor slave cylinder
 
Thanks to you all for your replies, is it a massive job to replace the clutch, and is there any literature on the site from anyone doing the job?
