P38 Lumpy Tickover/Throttle problems

I have a few problems with my P38 diesel (manual box).

First problem;
The engine sometimes gets very lumpy on tickover and a blip of the throttle cures it for a short while.

The second problem;
I've done a search before posting this question and found the following quote;

which is exactly the problem I have except there was no answer in the thread I found that quote on. This quote was found on a thread about lumpy tickover as in my first question. So guess the problems are/could be related.

Third Problem;
One other problem is on occasion after a gear change (manual box) the throttle pedal does not work, if you shut the throttle and apply it again its fine.

After doing the search I read it could be the EGR valve but my 1998 car does not have one fitted. Or possibly the Dual Mass Flywheel which I have already replaced so don't think its that. One other thing is there is sometimes quite a bit of black smoke on hard throttle, not sure how much is considered normal on an Diesel Range Rover ??

The lumpy tickover can be caused by a malfunction of the fourth injector. It has obviously not failed all together or it would be perminent, clean the contacts in the plug and see what you get.
 
i know, its not helpful but after my recent spate of accident/illness i have loads of frustrated comments trying to work thier witty way out!!!
