Hub nuts untightening

I am having this problem at the moment. Its really hit and miss, because the inner nut cant be tightened too much because it will crush the rollers, but the outer one cant be done up enough! My box spanner is the type where you use a screwdriver shaft as a turn, through the side.

I always bend the tab washer nice and tight over each way. Am I doing the inner up too loose?
 
It's been a while since I was doing Series wheel bearings, but is there a spacer piece between the inner nut and the outer taper roller bearing? If that is missing then you might experience the issue you have there.

Can't think of any other reason for it.....tricky thing to diagnose remotely :)
 
You fit the inner nut, then fit the tab and bend 2 sides over the inner nut (leaving the others flat). This should hold the inner without it turning. Then you tighten the outer to the torque required, then bend the tab over 2 of its sides.
 
That's where I've gone wrong. I've bent it one way then the other. How could I put torque on a side turn box spanner?
 
I do the inner one up tight, then back it off one flat of the hub spanner, so one sixth of a turn, then put lock tab on.
