free wheel hub

hi all

I have a series 3 109 pickup with the the two lever type free wheel hubs in the front. While driving the bolts on the left free wheel hub push out and the cover with the 2 levers fell off. i have been hearing different stories as to why this happened, eg broken side shaft, bent side shaft or broken universal joint.

Please help by advising why this happened.
