Everything needed for a clutch replacement?

I've a got 300TDI 1996 Defender 90. My clutch is on it's way out (it's done 200000 miles mind).

I dont think this is something I'm going to tackle myself to I'll take it to a garage to get it done, but before I start asking around for some quotes I just want to know exactly what I need to ask for.

Obviously I know I need the clutch, I've read that people have said about getting the clutch fork replaced at the same time, so I would probably do that. What about a flywheel? Is it worth getting that with the clutch?

Is there anything I've missed?

I've got a leak from my gearbox so I'd ask them to diagnose that as well and potentially repair.

Extra points:
Can anyone reccomend a clutch? (I mainly do motorway and town and city driving).
What kind of price would I be looking to pay for a garage to do all of this?
 
I have just done my clutch, as a fitted a recon gearbox, the chance of needing a flywheel is vitually zero, my engine has 307k on it and has not needed a replacment. I used the below:

UQB000130G - 130 (HD) clutch plate
URB100760G - clutch cover
FTC5200HD - HD release bearing (considered the LOF version but price was too steep for me)
FTC2957HDKIT - HD release fork kit (fork, slipper pads, plastic clips, spigot bush)

That is £172 worth of parts inc vat from paddocks where I ordered. The last time I was trying to get a garage to quote for a land rover clutch (not for a while as I do it myslef now) I struggled to find someone who was willing to do it do to the size and weight of the job.
