200TDI Disco Engine in 1990 Truck Cab.

About a month ago I had problems with water getting in to the oil, not a problem just drop the head off an pop in a new head gasket, and that is what I did, I even had the head skimmed to ensure all was good.

Well earlier this week I checked the oil and water and it wasn't looking good, water missing out of the radiator (most of it), and nice looking mayonnaise under the oil filler cap.

I have had a new radiator, all water pipes, heater radiator and thermostat. All the breather pipes were taken off and cleaned out, and a new cyclone filter was fitted sump dropped off and all cleaned out, this was done to remove as much of the contaminant the first time around, so I wouldn't have thought that it is residue from first time around.

I am expecting the worst but any ideas?
 
I had an issue with mine, replaced the headgasket without a skim and test and it carried on loosing water, thought it was going to a cylinder so no creamy oil. Jumped in at the deep end and got a second hand head which I swapped with another new gasket and it carried on loosing water then I noticed a slight dribble at the bottom on the engine side of my rad, replaced the rad and to my surprise it carried on loosing water so I had my original head tested and skimmed which came back perfect so I swapped the head again and that cured it. Original fault for me was dodgy radiator and I caused a second fault by fitting a head that I should have had pressure tested
 
Well I have started the strip down on it again and taken the head off again. The head gasket had stuck to both the head and block and came to bits when I separated them leaving a lot of it on the block, I don't know if this is normal but the original came off in one piece. I used an Elring replacement so don't know why it almost looks like it has failed? I am going to get the head pressure tested this time just to make sure.
 
Has anyone seen anything like this on a head gasket that has only done between 200 and 300 miles since fitting?

I had the same on earyy 2 litres diesel that I fitted later 2.25 gasket to, it did approx 1k before coming off again and was a right pig to clean off the faces.
 
it doesnt look from those pics that gasket was leaking though its not possible to rule it out, id be looking for another cause

Well I have had the head pressure tested and all checked today and there is nothing wrong with the head. The radiator and oil cooler is all new but I will pressure test that this weekend to eliminate that. Then I don't know where to go?
