Indeed as the error is at low revs would check the primary turbo actuator, also ensuring the wiring to it is ok , the secondary only kicks in over 2700 RPM.
If u don’t mind me adding Plse be very careful if ur not sure on some resets etc as u can brick the car
Might be an idea to have someone drive the discovery while u watch the live data , turbos etc , taking screenshots of the data , at least then ur more info to go on

Ps, there’s also a turbo oil drain modification kit and if the actuator is stuck and keeps doing it they have an updated one that u can buy
