I am 99.7654% sure all your problems are down too a cracked vacuum hose. Do not bother trimming the ends, just replace the whole lot for what it costs. TWEETS if you have a diesel your vacuum pump is located under the tray for the jack (i hope you have small hands !!!)

thanking u 2On my V8 4.6 I had a split in the plastic TEE section, I found this when I was about to change the hose.
The foot switch and bellows are known to cause trouble as well.
