Email - [email protected], give them the index and they should be able to help you out. Its a called a Merlin Report. I've not tried this, but i've been told to do this, they also require you to send them a copy of a household utility bill!?

I didnt have to send a copy of my household bills. Cant think why you would need to?
Make a freedom of information request to the MOD using that email, or write to them. Include your address so they can post it back to you.
