From what I can see of you photos you will be pushing the flange back on with that setup. That may be why you breaking bolts!

There is normally a filter in the oil feed pipe to a turbo. It is not subject to routine maintenance, but should be available as it should be replaced as a matter of course when the turbo is replaced. Unusual for it to be the cause of a turbo failure though, has it been serviced regularly?

Found the issue the clutch friction plate has fallen apart, looks like its done well though its date stamped 1992 so I assume its the original!
