I would start by having a good look under the truck. The exhaust runs very close to the gearbox and there is a chance it is just this rattling or something close by. If not it may be part of the hi/lo diff lock change mechanism that has come loose and is knocking against the gearbox. Not sure...

Sorry I cannot help you with your misfire problem but I am sure someone here will. Just like to comment that your English is excellent. My only suggestion is that in my experience, intermittent, cold running problems are usually due to loose or damaged connections in the wiring. Cheers Andy
