I looked at a W-reg D2 (older than I really want) yesterday. It had a replacement refurbished engine in 2016.
The advisories on the MOT's since then both say "Emissions too low".

Is this an indication of something terminal or just critical?

Most of the chassis didn't look too bad, and I suppose the gaffer tape and holes on the bumper could be sorted or lived with.
 
Here's the actual wording - I had it slightly wrong. The Odometer reading was, according to the selling garage, in kilometres!! It is now set to read miles -118k. Not sure how much I trust the testing station or the selling garage.

To clean is never going to be an issue, if you google it Iirc its 3.00 for a turbo diesel.
If its starts and pulls well dont worry about the engine the rest of the car especially the chassis/bulkhead is the biggest worry by a long way.
