Discovery 3 problem?

Hi everyone
I have a 2007 tdv6 discovery 3
After I do around 10 mile it decides to as if it turns it self off but doesn't if you understand then I get limp mode and special programmes off

Soon as I try accelerate hard it does it again it seems to be worse when the landrover is warm.

I've changed fuel filter brake switch

Any advise would be great
